
Anorexia nervosa is a psychiatric illness characterized by a persistent restriction of food intake, an intense fear of gaining weight, and a distorted perception of one’s body. Its roots extend far into the past. The disease is most common in adolescent girls and young women, but affects people of all genders, ages, and backgrounds. Boys, men, and older adults are underdiagnosed, partly because of cultural stereotypes leading to missed warning signs in these patients.
In 1873, the English physician Sir William Gull described several young women who had become profoundly emaciated despite the absence of any identifiable physical disease. He coined the term anorexia nervosa, emphasizing that the disorder arose from the nervous system rather than from the digestive tract. Independently, the French physician Ernest-Charles Lasègue also described the syndrome.
Most anorexia nervosa patients do not lose their appetite. They experience hunger, sometimes intensely, but consciously suppress it. Food becomes both feared and obsessively contemplated. The illness is not one of absent hunger but of relentless self-denial. Weight loss is generally achieved by limiting food intake, but sometimes it is achieved by binge eating followed by self-induced vomiting (more commonly known as bulimia) or misuse of laxatives.
The causes of the disease are complex. Studies of twins and families suggest a strong hereditary component, and researchers have identified differences in brain structure and neurotransmitter function, particularly in serotonin and dopamine pathways, that may predispose individuals to the disorder. No single one has yet been found to cause the illness; rather, many genes combine with environmental influences to produce vulnerability. Psychiatric studies have revealed important abnormalities, and many patients literally perceive themselves as overweight despite obvious emaciation.
Environmental triggers, such as major life transitions, trauma, bullying (especially about weight), or participation in activities that emphasize a lean body type (such as ballet, gymnastics, or running) can also increase vulnerability. Perfectionism, high levels of anxiety, obsessive thinking patterns, and difficulty tolerating uncertainty are common. For some, restricting food becomes a way of exerting control in a life that feels chaotic or overwhelming. Societies that idealize thinness and equate it with success, discipline, or beauty create an environment where disordered eating can take root.
Starvation itself produces profound biological changes that reinforce illness. As caloric intake falls, metabolism slows dramatically. Heart rate declines, blood pressure falls, body temperature decreases, and hormonal systems shut down to conserve energy. Menstruation frequently ceases. Bone density diminishes, predisposing young women to osteoporosis. The heart muscle weakens, electrolyte disturbances develop, and fatal cardiac arrhythmias may occur. The brain itself is not spared; cognitive functioning, concentration, and emotional regulation can all be impaired by starvation. Death may result from cardiac complications, overwhelming malnutrition, infection, or suicide, and mortality exceeds that of nearly every other mental illness.
The cultural environment plays an undeniable role. Throughout much of human history, plumpness symbolized prosperity because food was scarce. During the twentieth century, however, especially after the 1960s, ideals of beauty shifted dramatically toward extreme thinness in many Western societies. Fashion models, advertising, film, and later social media increasingly celebrated slender bodies that were often unattainable for most individuals. These cultural pressures appear to trigger illness primarily in those who are biologically and psychologically susceptible.
Family dynamics were once believed to play a role, blaming controlling mothers, emotionally distant fathers, or dysfunctional family relationships. Modern research has largely abandoned the notion that parents cause anorexia. Instead, families are increasingly viewed as essential partners in recovery.
Treatment remains challenging. The priority is to restore adequate nutrition, particularly when weight loss threatens life. Hospitalization may be necessary for severe malnutrition, dangerously slow heart rates, or electrolyte abnormalities. Nutritional rehabilitation must proceed carefully to avoid refeeding syndrome, a potentially fatal metabolic complication that can occur when severely malnourished patients receive calories too rapidly.
Medical stabilization and nutritional rehabilitation are a priority, particularly in severely malnourished patients. Psychotherapy has shown effectiveness in helping individuals challenge distorted thoughts about food and body image while developing healthier coping mechanisms. Because anorexia often thrives in secrecy and isolation, connection—whether through loved ones, support groups, or peer recovery communities—can be a powerful counterforce to the illness. Family-Based Treatment, often called the Maudsley approach, is one of the most effective therapies for adolescents, empowering parents to supervise eating until a normal weight is restored. Cognitive behavioral therapy helps older adolescents and adults identify distorted beliefs regarding weight, body image, and self-worth. While medications have an important role, no drug has been shown to be consistently effective.
Recovery is often slow. Some patients recover completely, others experience periods of relapse, and a few develop a chronic illness lasting decades. One of the greatest obstacles to recovery is stigma. Anorexia is often trivialized and can lead one to underestimate its seriousness. It is a multifaceted illness, shaped by genetic predisposition, psychological vulnerability, and cultural pressures, and it carries profound risks to physical and mental health. With early recognition, appropriate medical and psychological treatment, and a supportive environment, recovery is achievable. The disease straddles the intersection of psychiatry, endocrinology, nutrition, neuroscience, and social science, illustrating how closely mind and body are intertwined.
